SLS is a multi-award-winning supported housing provider specialising in services for individuals with complex mental health needs, learning disabilities, autism, and other support requirements. We are committed to challenging stigma and empowering people to live independent, fulfilling lives.

We are currently seeking an experienced and motivated Referrals and Assessments Coordinator to join our dynamic Business Team. This is a fantastic opportunity for a developing professional looking to build their career in supported housing referrals and service development.

Key Responsibilities

  • Referrals and Assessments
    • Act as the first point of contact for all incoming referrals from local authorities, NHS trusts, and partner agencies.
    • Conduct comprehensive needs-based assessments (can be face-to-face or via Microsoft Teams depending on the assessment needs) to evaluate mental health needs, risks and safeguarding concerns, daily living support requirements, and compatibility with SLS service models across the various operational locations (East London, Greenwich, Wandsworth and East Sussex).
    • Assess client readiness for supported living and suitability for available placements.
    • Ensure all referrals meet SLS eligibility criteria and internal governance standards.
    • Work closely with the business manager for voids and new locations.
  • Referral Management and Coordination
    • Process referrals in line with SLS internal protocols, maintaining accurate and up-to-date records.
    • Coordinate internal discussions with operations, quality and compliance, finance, housing, and support teams.
    • Track each individual's journey from initial referral to move-in and step-down, ensuring continuity and clear communication.
    • Liaise with commissioners and care managers to manage expectations, timescales, and outcomes.
  • Reporting and Documentation
    • Produce clear, detailed assessment and recommendation reports to support placement decisions.
    • Ensure documentation complies with Care Quality Commission standards, local authority commissioning requirements, and SLS quality frameworks.
    • Maintain confidentiality and data protection in line with GDPR and organisational policy.
  • Stakeholder and Partnership Working
    • Build and maintain effective working relationships with local authorities, NHS trusts, Integrated Care Boards, and voluntary sector organisations.
    • Represent SLS professionally in multi-disciplinary meetings, reviews, and planning discussions.
    • Act as a consistent point of contact for families and professionals throughout the referral process.
  • Marketing and Brand Representation (Supportive Role)
    • Promote awareness of SLS services to external stakeholders in line with company values and brand guidelines.
    • Support referral growth through networking events, service presentations, and information sharing.
    • Ensure all external communication reflects SLS's commitment to person-centred, high-quality supported living.
  • Business Development Support
    • Monitor referral trends, outcomes, and placement success rates.
    • Provide insight and data to support service development and commissioning discussions.
    • Contribute to evaluating referral and marketing activity to ensure value for money and service effectiveness.
    • Support, but do not lead, contract discussions and partnership engagement.
  • Quality, Safeguarding and Compliance
    • Identify and escalate safeguarding concerns in line with SLS safeguarding policies.
    • Ensure assessments are completed using a person-centred and strengths-based approach.
    • Work in accordance with mental capacity legislation, best interest principles, and risk management frameworks.
    • Promote continuous improvement in referral and assessment processes.

Requirements

Essential:

  • A good understanding of all mental health conditions, learning disabilities, and autism.
  • Experience in conducting needs-based assessments or triage interviews.
  • Strong written and verbal communication skills.
  • Highly organised with excellent attention to detail.
  • Good time management and task prioritisation.
  • Strong interpersonal and relationship-building skills.
  • Confidence working independently and collaboratively.
  • Able to travel nationwide to conduct assessments.

Desirable:

  • A full UK driving licence with access to a car.
  • Experience in marketing, referrals, assessment or business development in a healthcare or social care setting.

Experience (Required)

  • Referrals and assessments: 1 year
  • Marketing or communications: 1 year
  • Mental Health and Learning Disabilities: 2 years
  • Degree in Psychology or similar

Enhanced DBS check is required for this role. Candidates not on the DBS Update Service will have to meet the cost for a DBS Check.
