Accountable to: SOL Connect Team Leader

Salary: £14.35 per hour

Hours: 42 average per week

Annual Leave: 6 Weeks

Base: Wishaw

Work Pattern: Shifts are 12 hours, 8.30pm – 8.30am.

WEEK 1 - Tuesday, Wednesday, Thursday (36 HOUR WEEK)

WEEK 2 - Monday, Friday, Saturday, Sunday (48 HOUR WEEK)

General Description

SOL Connect is an award-winning Technology Enabled Care (TEC) service that helps people live safely and independently in their own homes. By combining smart technology with person-centred support, we help people stay connected, in control and able to access support when they need it.

Through the SOL Connect HUB, which enables face to face video conferencing, we provide 24/7 real-time support, reassurance and connection, helping people live the lives they choose within their own communities.

Job Purpose

As a SOL Connect Responder, you will provide overnight support to people using our service, helping them remain safe, independent and connected within their own homes. This isn't a traditional support worker role. Using our SOL Connect HUB you'll connect directly with people in their homes from our office base, providing reassurance, wellbeing support, medication prompts and other monitoring. Sometimes people need in-person support through the night, and that's where you make a difference.

As a member of our Responder team, you will play a crucial role in helping people live safely and independently in their own homes. By responding when support is needed most, you will help prevent avoidable hospital admissions, enable safe hospital discharges, support unpaid carers, and improve the quality of life for people who rely on care and support services.

Key Responsibilities

  • Deliver person-centred support in line with individual outcomes and Technology Enabled Care (TEC) support plans.
  • Build trusted relationships with people using the SOL Connect service.
  • Use the SOL Connect HUB to provide responsive support, reassurance and wellbeing monitoring.
  • Assess situations and make informed decisions to ensure people receive the right support at the right time.
  • Carry out remote and in-person wellbeing visits when required.
  • Support people to remain safe, independent and connected within their communities.
  • Respond to changes in wellbeing, requests for support and incidents in line with agreed procedures.
  • Support people following a fall or other incident in accordance with training and organisational procedures.
  • Liaise effectively with families, carers, emergency services and partner agencies when required.
  • Respond to issues relating to TEC equipment and escalate concerns where appropriate.
  • Promote safeguarding, dignity, choice and control in all aspects of your work.
  • Maintain accurate and up-to-date records using organisational systems.
  • Contribute to the ongoing development and improvement of the SOL Connect service.
  • Act as a positive ambassador for SOL and its values.
