Job description, key purpose
This role is a key part of the commissioning team which is funded by Southwark Council and SEL ICS.
The role leads a commissioning team focused on older and/or frail residents, including physical disabilities, of Southwark.
The post holder will have a significant role in building working relationships and collaboration across the council, the wider health system and through Partnership Southwark.
As part of this team the post holder will lead and therefore significantly contribute to developing and delivering a specified programme of work which will focus on a defined population within Southwark.
The post holder will be expected to contribute to the professional development of the team and co-ordinate the delivery of a complex programme of work centred on core health and care priorities in Southwark and South East London.
The post holder will also significantly contribute to planning for continuous reduction in health and wellbeing inequalities across the borough in collaboration across the council, the wider health system and through Partnership Southwark.
