LOCUM Social Worker (Adult Services - Learning Disabilities Review Project) job in Somerset. About the Role

We are seeking an experienced Adult Social Worker to join a Learning Disabilities Review Project. This is an excellent opportunity for a practitioner with strong Care Act knowledge and Learning Disabilities experience to undertake detailed reviews and reassessments, ensuring individuals receive appropriate, person-centred, and evidence-based support.

The successful Social Worker will work independently while collaborating closely with health professionals, providers, families, and partner agencies to deliver high-quality assessments and defensible decision-making.

Key Responsibilities

  • Undertake detailed Care Act reviews and reassessments for adults with learning disabilities.
  • Conduct face-to-face visits within homes, supported living services, residential settings, and community environments.
  • Gather and analyse information from individuals, families, providers, advocates, health colleagues, records, and direct observations.
  • Determine whether a statutory review or full reassessment is required and provide a clear professional rationale.
  • Identify and evidence eligible care and support needs in line with the Care Act.
  • Produce high-quality, evidence-based, audit-ready assessments, reviews, support plans, and case recordings.
  • Support and contribute to CHC processes, funding reviews, and multidisciplinary discussions where required.
  • Work collaboratively with Adult Social Care teams, health colleagues, providers, and stakeholders to achieve timely outcomes.
  • Maintain accurate electronic records and ensure compliance with legislation, guidance, and best practice.

Essential Requirements

  • Social Work England registration with significant post-qualifying experience
  • Strong Learning Disabilities practice experience
  • Experience completing Care Act reviews and reassessments
  • Excellent assessment, analytical, and evidence-based decision-making skills
  • Ability to identify eligible care and support needs under the Care Act
  • Experience working within multi-disciplinary teams and partnership settings
  • Knowledge of CHC processes, Checklists, DSTs, joint funding arrangements
  • Experience challenging funding decisions
  • Ability to work autonomously, manage workload effectively, and deliver on time
  • Strong IT skills
  • Full UK driving licence and access to own vehicle and ability to travel

Desirable Experience

  • Experience working with adults with Learning Disabilities across a variety of settings
  • Supervisory experience
  • Understanding of social care resources beyond statutory services
  • Awareness of current legislation, government guidance, and developments affecting Adult Social Care
