Job Summary
Each day we deliver the health and social care services that people in Northamptonshire receive, in the community, at home, work or in schools. The \”Recovering Independence Bed\” unit is delivered by a range of staff from health and social care settings to provide clinical care, rehabilitation support, and care support, ensuring patients experience the best quality of service they can in a prompt and smooth fashion. This role provides a unique opportunity to work with a dedicated team, with experience from both health and social care, to deliver vital care for patients in Northamptonshire. You will provide quality care specialising in rehabilitation/reablement and sub-acute care for patients stepping down from acute hospitals. As the focus of the centre is rehabilitation and reablement, you will work in an integrated multi‑disciplinary team that includes senior nurses, staff nurses, unregistered staff, physiotherapists, occupational therapists, medical staff and social care colleagues.
Main Duties
- Assess the healthcare needs of patients and develop care plans reflecting their needs
- Monitor the quality of care they are receiving
- Work closely with the MDT to set goals towards discharge
- Carry out clinical skills such as complex medication administration, intravenous medications and fluid administration, syringe pump use, enteral feeding, complex wound care and catheterisation
- Assist the senior nursing staff in managing a team of patients, supervision and delegation to junior staff
- Be open to new ways of working in the centre and developing strong links with other services and the wider community
- Be well organised, confident and adaptable to deliver high‑quality patient care
- Lead and manage effectively, using communication skills, problem‑solving and good clinical judgement
- Work flexibly with minimum supervision, prioritising work based on patient need
- Possess IT skills
About Us
The Recovering Independence Beds Unit has 51 beds and provides care for patients across three wings. Patients may require more nursing input as part of their recovery, or they may be recovering from delirium associated with their condition or related to an acute onset, or they may be admitted as a stepping stone awaiting a social care package. The post holder is expected to support or work in all three wings depending on service need. We are developing an in‑house NHFT medical model to support the multidisciplinary team. This post is offered predominantly at night; the post holder will work a pattern of days agreed with the Matron.
