The Hospice at Home Team is part of the Community Specialist Palliative Care Team across Northamptonshire. It provides holistic nursing care for end-of-life patients in their own homes, over a 24-hour period. We care for patients requiring symptom management and end-of-life care at home. Excellent communication and interpersonal skills are essential, as you will need to communicate sensitively and compassionately with patients nearing the end of life and their families.
Experience in community settings and palliative nursing is ideal, but a passion for providing outstanding holistic nursing care is essential.
This 24-hour service requires flexibility to varying shift patterns, including early, late, and night shifts, as well as rotational weekends and bank holidays. Travel across Northamptonshire to patients\’ homes is required.
The Post Holder will play a vital role in providing holistic care tailored to the needs of patients and their families, often without direct supervision. Responsibilities include:
- Assessment of needs and symptoms, and implementation of individualized care plans.
- Providing information and involving families in care planning and evaluation.
- Administering prescribed medications as required.
- Maintaining accurate records.
- Ensuring effective team communication and coordination with other community services.
- Providing sensitive support to patients\’ loved ones.
We seek a nurse with resilience who can manage the emotional and physical demands of caring for patients in their last weeks of life. The role may also involve supporting the learning and development of students and clinicians.
