Responsibilities
- Identify ongoing needs and services of residents to promote high-quality care.
- Ensure proper follow-up, participate in evaluations, and document changes in residents’ health status.
- Assist Resident Care Director (RCD) in conducting evaluations and data collection at move-in, 30 days post-move, semi-annually, and with any change in status.
- Conduct monthly wellness visits for all residents.
- Monitor and follow up on residents who experience falls, accidents, or incidents; document and report accordingly.
- Document all pertinent information in residents’ wellness files.
- Maintain communication with residents’ families or responsible parties to ensure medical needs are met.
- Contact attending physicians when necessary or upon family request.
- Ensure weights and vital signs are obtained monthly for each resident.
- Assist in maintaining wellness files according to policies and regulations.
- Maintain medical supplies and emergency kits.
- Provide clinical support to team members as needed.
- Follow infection control practices and promote standards including testing, training, and skills checks.
- Handle and dispose of blood-borne pathogens, bodily fluids, and hazardous materials safely.
- Monitor medication and treatment profiles to ensure accurate administration and documentation.
- Participate in quality improvement areas such as Resident-Centered Care, APIE, and Quality Care Indicators.
- Understand and adhere to federal, state, and local regulations and resident rights.
Qualifications
- Graduate of approved nursing program.
- Current state license as an LPN/LVN.
- At least one year of experience in relevant healthcare settings.
- Knowledge of nursing practices, regulations, and assessment skills.
- Effective communication and organizational skills.
- Proficiency in computer skills including Microsoft Office and Sunrise applications.
- Ability to work flexible hours including weekends and evenings.
