Job Overview
The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/longāterm care and reminiscence neighborhoods. The Care Manager provides handsāon care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP), while maintaining a safe, comfortable, homeālike environment. The Care Manager demonstrates Sunrise's mission to champion quality of life for all seniors in accordance with federal, state/provincial, and local laws and regulations, and Sunrise Senior Living standards, policies, and procedures.
Responsibilities & Qualifications
- Review, notate, and document Daily Log to capture pertinent resident information.
- Participate in developing and updating ISPs.
- Monitor designated residents throughout the shift, and report changes in condition.
- Respond to security systems and resident call bells promptly.
- Assist with continence management and hygiene.
- Communicate regularly with families as a resource.
- Help residents maintain independence, safety, and dignity.
- Participate in activities of daily living (ADLs) and encourage resident participation as indicated on the ISP.
- Engage residents in life skills and enrichment activities throughout the day, especially in reminiscence.
- Maintain a warm, calm environment that adheres to safety regulations and resident rights.
Risk Management & General Safety
- Ensure compliance with occupational health and safety requirements, including PPE, fire safety, and lockout/tagout.
- Report incidents and hazardous conditions immediately.
- Maintain safe storage of oxygen tanks and other equipment.
Housekeeping & Laundry Services
- Clean resident rooms and common areas, including bed making, trash removal, and surface disinfection.
- Wash and fold resident laundry as needed.
Dining Service
- Serve meals and support dining room operations.
- Ensure a pleasant dining experience and follow resident diets as per ISP.
- Assist with setup and cleanup and note any dietary changes in the daily log.
- Practice safe food handling.
Training & Team Success
- Participate in team meetings, training, and engagement surveys.
- Maintain compliance with infection control, bioāhazard protocols, and state licensing requirements.
- Perform additional duties as assigned.
