Sunderland Care and Support is one of the leading Social Care Providers of high-quality care and support to the community of Sunderland. We are passionate about supporting adults with Complex Learning Disabilities to lead fulfilling lives. We are seeking to recruit Care Co-ordinators (Level 1's) into our Autism Team who can deliver exceptional standards of person-centred care and support to those with a high level of physical support needs as well as those who may be at risk of displaying behaviour that challenges services.

Responsibilities
- Helping with preparation of meals and drinks.
- Support with domestic household tasks.
- Administer and record medication.
- Support with money management.
- Encouraging community trips and facilitating customer participation.
- Support with household and personal shopping.
- Support with meeting friends and family.
- Support with high standards of nutrition.
- Adhere to health and safety legislation.
- Cleanliness and intimate personal hygiene.
- Opportunities for employment or education.
- Provide emotional support.
- Active support techniques and positive behaviour support techniques to increase skill level and achieve set outcomes (training will be provided).
- To support customer to plan holidays and support them if needed (additional payment).
