Role: Care Co-ordinator (Level 2) within the Mental Health Team, responsible for delivering person-centred care and support to customers.
Responsibilities:
- Completion of support plans
- Various audits and lead the team in the absence of the team leader
- Minutes of meetings
- Competent use of IT
- Help with preparation of meals and drinks
- Support with domestic household tasks
- Administer and record medication
- Support with money management
- Encourage community trips and facilitate customer participation
- Support with household and personal shopping
- Support with meeting friends and family
- Support with high standards of nutrition
- Adhere to health and safety legislation
- Maintaining cleanliness and intimate personal hygiene
- Facilitate opportunities for employment or education
- Provide emotional support
- Apply active support techniques and positive behaviour support techniques to increase skill level and achieve set outcomes (training will be provided)
- Support customer to plan holidays and provide additional payment where required
