Annual Salary: £40,000 - £42,000 plus monthly bonus scheme and Car Allowance

Contract Type: Full Time (40 hours per week)

Office Times: 07:30-16:30 Monday to Friday

As a Nurse Care Manager you will be responsible for the operational day-to-day management of the service, ensuring our continued compliance with relevant legislation. Working in partnership with colleagues and the senior management team you will ensure the provision of a safe, high quality service enabling individual needs.

Responsibilities

  • Lead on the assessment, planning and review of bespoke care packages for adults with complex health needs, ensuring all care plans and risk assessments are completed to a high clinical standard.
  • Provide clinical leadership, supervision and competency assessments for Healthcare Support Workers, ensuring they are fully trained and able to deliver safe, effective care.
  • Monitor and promote clinical excellence and compliance, maintaining adherence to NMC standards, organisational policies, and regulatory requirements across all assigned cases.
  • Collaborate effectively with multidisciplinary teams, including Registered Managers, Care Coordinators and external health professionals, to ensure a cohesive and person‑centred approach to care delivery.
  • Provide ongoing clinical support and respond appropriately to urgent or emergency situations, ensuring service user's needs are met safely and promptly.
  • Deliver high‑quality training and education, developing and adapting materials to enhance staff skills and improve outcomes for service users with complex and changing health needs.

Clinical Requirements

  • An Adult Registered General Nurse with a current NMC PIN Number.
  • Experienced in ICU or complex needs, such as tracheostomy, PEG & gastrostomy or ventilated service users.
  • Experience in developing and delivering personalised health and care plans.
  • Experience in supervising or delegating nursing care tasks.
