About the role: You’ll provide specialist, person‑centred care to a young service user with complex needs in a home setting. The role involves delivering safe clinical and social care in partnership with family and professionals.
You will be required to:
- Provide continuous observation and monitoring
- Support airway management and oxygen therapy, responding promptly to changes in saturation levels
- Deliver enteral feeding, hydration, and medication safely via gastrostomy
- Assist with personal care, hygiene, and continence support
- Support safe mobility and manual handling, following specific guidance to prevent injury
- Carry out repositioning throughout the night to maintain airway safety and skin integrity
- Monitor and record vital signs, input/output, and clinical observations
- Support pain management, recognising non‑verbal cues and escalating when necessary
- Maintain accurate and timely documentation and reporting

Requirements for the Role:
- A minimum of 6 months of professional UK care experience.
- Must hold a full UK driving licence.
- Excellent communication skills and the patience to build a strong, understanding relationship with an individual who has full mental capacity and can make his own choices.
- The ability to follow detailed care plans and protocols meticulously.
- A proactive and observant nature, able to spot subtle changes in an individual’s condition.
- To provide the correct compliance documents, including an updated Right to Work and a valid DBS certificate.
