Little St John’s Surgery are looking for a Care Navigator to join our Reception team in Woodbridge. You will need to be flexible, highly motivated and friendly with an excellent eye for detail. The post offers 22 hours a week as follows:

  • Monday PM 13:00 to 19:00
  • Tuesday AM 08:00 to 13:00
  • Thursday PM 13:00 to 19:00
  • Friday AM 08:00 to 13:00
  • Plus occasional Saturday mornings and cover for holidays and sickness as required.

Any employment offer will be subject to a full DBS check and References.

Job summary:

Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. Provide general assistance to the practice team and project a positive, professional and friendly image to patients and other visitors, either in person or via the telephone.

PRINCIPAL DUTIES AND RESPONSIBILITIES TO INCLUDE:

  • To have a thorough knowledge of all practice policies and procedures.
  • To work in accordance of written protocols.
  • Processing all post and electronic communications on the day they arrive.
  • Managing the uploading and downloading of temporary resident forms.
  • To manage the waiting list for 24 hour blood pressure monitoring and 24 hour ECGs.
  • Any other Administration duties required as advised by the Team Lead.
