Your role and responsibilities:
- Support the Home First team with customer assessments and reviews.
- Work with allocated customers to identify and deliver high-quality, person-centred outcomes.
- Build strong relationships with customers to achieve personalised goals and improve independence.
- Work collaboratively with social work teams, health partners and the voluntary sector to support integrated care.
- Make a meaningful difference to the lives of residents across Suffolk.
- Receive regular support and professional supervision as part of a collaborative team environment.

- The ability and energy to respond to urgent work as part of a team system.
