Your role and responsibilities:
- Complete assessments and reviews with customers to understand their needs and goals.
- Work directly with individuals to identify personalised outcomes and support them to achieve greater independence.

- Collaborate with colleagues across health, social care, and the voluntary sector to provide joined-up support.
