Your role and responsibilities

We’re looking for a motivated and enthusiastic practitioner to join our West Optimising Care and Support team. Working closely with colleagues across the organisation and our wider partners - including Health, Business, Finance, Service Development and Contracts teams - you’ll play a key role in delivering high-quality, person-centred support. The purpose of the role is to work with adults to identify strengths, needs and outcomes, supporting them to maximise their independence and wellbeing.

  • Work as part of a team to promote personalisation, independence, wellbeing and choice, carrying out asset-based assessments and reviews.
  • Develop and implement reablement plans using a strengths-based, solution-focused approach.
  • Contribute to the duty rota, responding to new referrals and urgent work in a timely way.
  • Work collaboratively with colleagues and partners to deliver coordinated, high-quality support.
  • Support individuals to make informed decisions, offering advice, guidance and signposting as appropriate.

You will need

  • Experience of working with adults, including those with complex needs.
  • Knowledge of Adult Social Care and a person-centred, strengths-based approach.
  • Ability to manage and prioritise a varied workload effectively.
  • Strong communication skills, with the ability to build relationships and adapt your approach.
  • Confidence using IT systems and maintaining accurate records.
  • Ability to work collaboratively as part of a team.
  • Resilience and a positive, proactive approach.

The team

The West Optimising Care and Support team play a vital role in supporting adults as they transition from hospital settings back into the community. Working collaboratively across health and social care, the team delivers a diverse and fast-paced service. The team is dynamic and responsive, adapting to fluctuating demand across the system. During periods of peak activity, team members may also work flexibly across locality teams and West Suffolk Hospital teams, providing additional support where it is most needed to ensure continuity of care.
