Job Description

  • Suffolk County Council – West Suffolk Hospital, Bury St Edmunds, Suffolk, IP33 2QZ – Hybrid
  • £33,366 per annum (pro rata for part time)
  • Post 1: 37 hours per week
  • Post 2: 18.5 hours per week
  • Permanent

Your role and responsibilities

As a Senior Independence and Wellbeing Practitioner, you\’ll be based at West Suffolk Hospital, working in a fast-paced acute hospital environment. Your focus will be on supporting timely and safe discharges, helping people regain their independence and wellbeing.

You will

  • carry out Care Act Assessments and Mental Capacity Assessments
  • assess functional ability and promote digital care technology
  • provide advice and guidance to customers, families, and health professionals
  • collaborate with NHS colleagues and social care teams to deliver joined-up support
  • support community referrals to prevent hospital admissions
  • manage referrals, including benefits tracking and eligibility decisions

You will also have the opportunity to work flexibly, including some home working. There is a requirement for weekend working, averaging 1 weekend in 5 which is shared across the team.
