Reablement Support Worker

- Salary: From £25,185 per annum, with pay progression up to £28,142 per annum (pro rata for part time)
- Hours: From 7.5 hours – 37 hours per week, with additional hours available
- Contract: Permanent
Your Role and Responsibilities
- Build and maintain positive relationships with customers and their family carers, promoting health, wellbeing, and independence.
- Support individuals through personalised care plans and short‑term enablement strategies, offering guidance to help them make informed choices.
- Assist with dignified personal care needs and engage with customers and carers to explore alternative ways of meeting long‑term needs.
- Promote services that enable safe, independent living at home, monitor progress against care plans, and maintain accurate records to support ongoing reviews.
