The role involves independently and effectively providing a professional Occupational Therapy Service with a highly complex caseload to promote independence, well-being and personalisation for adults with care and support needs living in their own homes, reducing the need for more intensive services including prevention of hospital admission. This includes:
- Carrying out high quality assessments of physical, psychological and environmental needs of clients.
- Recommending adaptation works to be undertaken using Disabled Facilities Grants.
- Helping develop the scope and services of the Independent Living – East Suffolk service in response to community needs, funding opportunities and Local Government Re-organisation.
What you will need:
- Good knowledge of health and care systems.
- Extensive knowledge and understanding of Disabled Facilities Grants and adaptations.
- Excellent organisational skills and ability to prioritise.
- Degree/Diploma in Occupational Therapy and a minimum of 5 years experience in a related, customer-facing field.
