Your Role and Responsibilities

As a Care Coordinator, your mission will be to support people in living well and independently at home. You will play a key part in transforming how care is delivered – by building strong, trusting relationships, streamlining services, and helping to reduce unnecessary hospital admissions. You’ll ensure care is delivered closer to where people live, making it more accessible, personalised, and effective.

  • Build trusted relationships with individuals and families to tailor support to their needs
  • Champion innovative projects and drive positive change
  • Work collaboratively with professionals across health, social care, and voluntary sectors
  • Navigate systems and services to ensure joined-up, responsive care
  • Enjoy autonomy, creativity, and the chance to make a meaningful impact
  • Identify gaps in service provision and contribute ideas to improve delivery
  • Support people through transitions in care, ensuring continuity and stability
  • Promote preventative approaches to reduce hospital admissions and improve wellbeing
  • Contribute to team learning and development by sharing insights and best practices

What We Are Looking For

  • A passion for helping others and strong communication skills
  • Confidence using digital tools and a willingness to learn
  • Experience in health, social care, or community settings is helpful – but transferable skills from other sectors are welcome
  • Ability to manage multiple priorities and remain calm under pressure
  • A proactive mindset with the confidence to take initiative and solve problems
  • Respect for confidentiality and sensitivity when handling personal information
  • A team player who values collaboration and brings positivity to the workplace

Details

Address: Hartismere Hospital, Eye IP23 7BH – Hybrid

Salary: £28,598 per annum (pro rata for part time) – Local Government Terms and Conditions

Alternatively, for existing NHS staff £27,485 per annum (pro rata for part time) – NHS Terms and Conditions

Hours: 21 hours per week, flexible working options available

Contract: Permanent
