Overview

Address: Hartismere Hospital, Eye IP23 7BH - Hybrid

Salary: £28,598 per annum (pro rata for part time) - Local Government Terms and Conditions

Alternatively, for existing NHS staff £27,485 per annum (pro rata for part time) - NHS Terms and Conditions

Hours: 21 hours per week, flexible working options available

Your role and responsibilities

  • Support people in living well and independently at home.
  • Build trusted relationships with individuals and families to tailor support to their needs.
  • Champion innovative projects and drive positive change.
  • Work collaboratively with professionals across health, social care, and voluntary sectors.
  • Navigate systems and services to ensure joined-up, responsive care.
