Responsibilities
- Have direct case responsibility for a cohort of pupils with EHC Plans in local mainstream and specialist settings
- Monitor progress through annual reviews and ensure EHC Plans remain current and relevant
- Draft and amend EHC plans ensuring these are of good quality and within statutory timescale
- Act as Lead Professional, coordinating input from other agencies as required
- Maintain accurate case records and provide reports
- Identify and escalate safeguarding/risk concerns
- Build relationships with partners in education, health, and community settings
- Provide high quality customer service in line with our WE ASPIRE values
Qualifications
- Graduate level qualification in a relevant field or equivalent experience
- Proven ability to build relationships and create a climate for positive change
- Excellent interpersonal, verbal and written communication skills
- Experience working directly with children, families and multi‑agency partners
- Commitment to safeguarding and promoting welfare of children and young people
- Strong organisational skills and ability to prioritise workload
