Our Clinical Nurse Specialist Team is based at our Theale office near Reading, but we welcome applications from nurses across Berkshire and are open to discussing flexible working options. At the Duchess of Kent Hospice in Reading and through our services in Newbury and Wokingham, we care for people living with life-limiting conditions and support their families. We provide expert care and support across Berkshire.

About the Role:
Sue Ryder invites applications from experienced nurses to join a friendly, well-established team as a Clinical Nurse Specialist in Reading. You will support patients with an end-of-life diagnosis to remain in their homes. This role is primarily telephone triage. Responsibilities include assessing referrals, performing comprehensive patient assessments, planning, implementing, and evaluating care, identifying potential palliative care emergencies, developing action plans, and ensuring effective communication with patients, families, and other professionals.
