Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
Job highlights:
- Each 45‑minute visit includes a medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry), based on the health plan’s requirements.
- Clinicians use an iPad and an intuitive clinical workflow for each evaluation.
- Clinicians are not required to prescribe medication, order laboratory tests, or alter existing treatment regimens.
In this role, you will:
- Build trusted relationships and work with a national network of purpose‑driven clinicians dedicated to improving health outcomes.
- Participate in Signify Health’s mission to visit millions of people nationwide, connecting them to the right care for their unique needs.
- Travel locally and within licensed states as required and conduct virtual visits as needed.
- Provide all Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).
A Note on Our Work Environment:
As an in‑home clinician, your primary workplace is the private residence of health plan members. Your duties may involve working in environments that contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to safely perform their duties in such settings.

The typical pay range for this role is $95,738.00 – $206,206.00. The actual base salary will depend on experience, education, geography, and other factors.
