Our services are contracted, and we currently have funding for this contract until December 2024. We are looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke Association to deliver our new Community Connect service in Carmarthenshire. This is an exciting role supporting and empowering stroke survivors and their families to be independent and benefit from opportunities in the community following a stroke.

The service aims to identify and address the needs of stroke survivors and carers once they leave hospital, by providing a range of innovative support solutions, enabling them to meet their desired outcomes.

The Stroke Association Community Connect Coordinator will:

  • Support new stroke survivors and their carers to live independent lives in the community.
  • Provide personalised information, advice and support.
  • Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
  • Work collaboratively with Social Care, NHS, 3rd sector colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
  • Promote stroke friendly communities.

This role requires the post holder to travel across the locality to connect people to their community and peers. Candidates must be able to demonstrate how they can meet this requirement of the role. To fulfil the role you must be resident in the UK and have the right to work in the UK.
