We are looking for a Practice Nurse who will work as part of the primary healthcare team within a GP practice, delivering high‑quality, patient‑focused nursing care. The role involves managing chronic diseases, delivering preventative care, supporting health promotion, and assisting in clinical procedures, in line with NHS standards and guidelines.
Key Responsibilities
- Provide general nursing services including immunisations and cervical screening.
- Undertake chronic disease management (e.g. diabetes, asthma, COPD, hypertension) in line with national and local protocols.
- Administer vaccines and injections safely and effectively.
- Perform health assessments, screenings, and NHS health checks.
- Support minor surgery procedures and assist GPs when required.
- Carry out travel health consultations and vaccinations where appropriate.
Patient Care & Health Promotion
- Deliver evidence‑based health education and advice to patients.
- Promote healthy lifestyles and disease prevention strategies.
- Support patients in self‑management of long‑term conditions.
- Maintain accurate, confidential patient records in line with GDPR and NHS policies.
Practice Support
- Maintain clinical equipment and ensure appropriate stock levels.
- Participate in infection prevention and control procedures.
- Contribute to clinical audits, quality improvement and QOF targets.
- Work collaboratively with GPs, healthcare assistants, pharmacists and other members of the multidisciplinary team.
Qualifications & Experience
- Registered Nurse (RGN) with valid NMC registration.
- Experience working in primary care or community settings.
- Competence in immunisations, cervical cytology.
- Knowledge of chronic disease management.
Desirable
- Additional qualifications in chronic disease management (e.g. diabetes, asthma).
- Experience with clinical systems such as EMIS.
