Advanced Nurse Practitioner - Acute Care & Home Visits (PT) in Wrexham

Strathmore Medical Practice in Wrexham is seeking an experienced Advanced Nurse Practitioner to join our clinical team for four sessions per week, to manage same-day demand and support patients within the community.

You will work closely with our GP team to conduct home visits, manage on-the-day acute presentations, and autonomously assess, diagnose and treat patients while using Klinik for triage; EMIS/DOCMAN knowledge is preferred, with ongoing professional development available.
