Responsibilities
- Assist patients with daily activities such as personal hygiene, dressing, and mobility
- Support the implementation of individualised care plans tailored to each patient's needs
- Monitor and record patients' health status, reporting any concerns to senior staff
- Provide companionship and emotional support to patients, fostering a positive environment
- Assist with medication administration following prescribed guidelines (training provided)
- Maintain accurate documentation of care provided using IT systems
- Support with light domestic tasks to ensure a clean and safe environment
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery
- Drive between different care locations as required, ensuring timely patient support
