We\’re looking for an experienced Registered Nurse CHC assessor to join a leading organisation delivering Continuing Healthcare (CHC) assessments. About the Role
Working within the National Framework for NHS Continuing Healthcare (CHC) and the Care Act 2014, you will:
- Chair Multi-Disciplinary Team (MDT) meetings held online (MS Teams or Zoom) with patients, families, and care providers.
- Complete CHC assessments and reviews using the Decision Support Tool (DST), providing clear eligibility recommendations.
- Create person-centred care and support plans.
- Liaise with local authority representatives and other professionals to gather evidence and ensure robust decision-making.
- Work to agreed Key Performance Indicators (KPIs) for quality, productivity, and service delivery.
This is a fully remote role, giving you the flexibility to balance professional and personal commitments while still making a tangible difference to people\’s lives.
What We\’re Looking For
We welcome applications from Registered Nurses (NMC-registered) with at least 12 months\’ post-registration experience and:
- Previous experience completing CHC assessments and reviews within the NHS or similar settings.
- Strong understanding of the CHC Framework 2022, Care Act 2014, and Mental Capacity Act 2005.
- Excellent written skills to produce clear, person-centred, evidence-based reports.
- Confident communication and empathy when dealing with sensitive, often complex health and social care matters.
- Ability to plan and organise workload, meet KPIs, and work collaboratively with other professionals.
