To play your part in ensuring that the team is effective and efficient in delivering its core purpose to the citizens of Stoke-on-Trent: Ensure that people are fully involved and empowered throughout their contact with the service. Ensure that the person is fully understood in order that the most appropriate tailored solutions can be offered. Identify and address any system barriers which prevent the people from experiencing the most efficient and effective journey through the service. Ensure that people are empowered, connected, independent and have an improved sense of well-being through the provision of outcome focused support. Meet with people regularly to discuss their experiences of the service to drive further improvement.

Main duties of the job

Responsibilities will routinely include Social Care assessments for adults and carers, reviews, care and support planning and continuing health care checklist and decision support tools.

Job responsibilities

  • Requirement to accurately maintaining client records and ensuring that these records are logged correctly on the departments IT system
  • Strong report writing and communication skills.
  • Relevant knowledge and experience in Adult Social Care either in a formal or informal capacity.
  • Able to fully meet the requirements of the post and cover the whole geographic area of the City either by possession of a current and valid driving licence or by access to public or other transport
