Our service is a busy, supportive environment made up of three integrated teams: Home First, the Integrated Discharge Hub, and the Community Bed Team. We are co‑located to provide a strong multidisciplinary approach, ensuring the best possible outcomes for the people we support.

Main duties of the job:

  • Undertake person‑centred wellbeing assessments
  • Work collaboratively with health professionals and partner agencies
  • Support timely hospital discharges and reduce delays
  • Manage a varied caseload within a 7‑day service

Job responsibilities:

  • Ensure that people are fully involved and empowered throughout their contact with the service.
  • Ensure that the person is fully understood in order that the most appropriate tailored solutions can be offered.
  • Identify and address any system barriers which prevent the people from experiencing the most efficient and effective journey through the service.
  • Ensure that people are empowered, connected, independent and have an improved sense of well‑being through the provision of outcome‑focused support.
  • Meet with people regularly to discuss their experiences of the service to drive further improvement.

Experience: Good understanding of personal emotions and behaviours and how they impact on work with citizens and partners; manage own behaviour and emotions to improve personal and professional effectiveness.
