There are three permanent positions available: 37 hours per week x 2 (Community Hospital Beds) and 37 hours per week x 1 (Home First Team).

The Hospital Social Work Service is a large, dynamic team operating across multiple locations. The service is made up of three integrated teams: Home First, Community Hospital Team, and the Integrated Discharge Hub based at UHNM. Each team plays a vital role in supporting timely discharges, promoting independence, and ensuring individuals receive the right care in the most appropriate setting. Working across these areas provides a varied and rewarding experience, with opportunities to develop skills in fast-paced hospital environments and community-based discharge planning.

Main duties of the job

  • Qualified Social Worker registered with Social Work England
  • Strong assessment, risk management and decision‑making skills

About You:

  • Proven ability to complete timely, high-quality Care Act assessments within a hospital setting
  • Experience of working effectively within a multi-disciplinary team, building positive professional relationships
  • Strong knowledge and application of the Mental Capacity Act, safeguarding processes, and risk assessment
  • Ability to manage complex and high-risk cases, making sound, defensible decisions under pressure
  • Commitment to strengths‑based, person‑centred practice, promoting independence and choice
  • Excellent organisational skills with the ability to prioritise workload and meet competing demands in a fast‑paced environment
