The Role:

An exciting opportunity has arisen to make a genuine difference to the lives of some of the most vulnerable members of the community of Stoke-on-Trent. Leading on the prevention and relief of homelessness and rough sleeping for people experiencing multipledisadvantage in the City you will lead a genuine multi-agency approach across the Council and its partners to deliver our objective of preventing homelessness and ending rough sleeping in the City, something that has recently been matched by the Government’s announcement of its National Rough Sleeping Plan. You will lead a team of officers working with people experiencing homelessness and rough sleeping to ensure effective delivery of pathways off the street, out of prison and following discharge from hospital in partnership with a wide range of internal teams, statutory and voluntary partners and commissioned services to ensure that no one leaves an institution in the City homeless and people supported off the streets access the support they need to ensure they do not return to rough sleeping. Demonstrating an understanding of the needs of people experiencing multiple disadvantage you will be a motivated, self starter with proved experience of building effective partnerships with a wide range of partners to deliver person centred solutions informed by lived experience.

Key Responsibilities:

  • Act as the lead contact for all multiple disadvantage, homelessness, rough sleeping and outreach services across the City
  • Build, develop and coordinate partnerships with internal teams and external agencies to support individuals experiencing homelessness and complex needs
  • Work collaboratively with housing, social care, health, mental health, substance misuse, domestic abuse, probation, voluntary sector and commissioned service providers
  • Chair and leadmulti agency meetings to coordinate support and develop effective plans for individuals experiencing homelessness or rough sleeping
  • Ensure support is in place for people living in temporary, emergency orcommissioned accommodation
  • Work closely with Housing Options and partner organisations to develop clear referral pathways and improve access to support services

About You:

  • Be a self starter and able to work on own initiative
  • Proven experience of working with people experiencing multiple disadvantage with emphasis on resolving homelessness
  • Experience in leading multi agency partnerships across a wide variety of services and agencies
  • Knowledge of the issues relating to the provision of services across the homelessness, adult social care, drug and alcohol treatment, health services including both physical and mental health and the criminal justice system and how these interact with people experiencing multiple disadvantage
  • Be confident in leading teams, liaising with senior officers across a wide variety of services, local members and representatives of national government
