Look no further! We have a fantastic opportunity for an ISPA officer to join our team here at Stockton‑on‑Tees Borough Council.

Contract Details

This is a fixed term contract covering maternity leave - the expected duration of the contract is 12 months but will be reviewed periodically with the post holder. This post is a 7 day working contract including evenings till 7pm on a rota system as the service requires. Currently staff are not rota’d on to evening working but this can change as the service requires.

Role Context

The ISPA Officer is part of the Integrated Intermediate Care Services based between Billingham Health Centre, North Tees hospital and home working, and is part of the Early Intervention and Prevention service. The key role of this post is to facilitate hospital discharges by working in an integrated team. The post holder will undertake the initial assessment processes and coordination of social care aspects of hospital discharges in accordance with Adult Social Care pathways for hospital referrals. This is an exciting opportunity to be part of a highly successful hospital discharge system. The ISPA team are committed to supporting safe and timely discharges from the local hospitals, working closely with colleagues from North Tees and Hartlepool NHS Trust.

As this is a fixed‑term post, secondment opportunities will be considered for internal employees. Please obtain prior approval from your current line manager before completing the application.
