We have a fantastic opportunity for a Social Care Officer to join our team here at Stockton-on-Tees Borough Council.
In this role you will:
- Support the social work and care management process for individuals with functional and organic mental illness and their families.
- Undertake assessments, reviews, and reassessments of care packages in line with departmental policies and procedures.
- Carry out tasks delegated by qualified social workers in more complex cases, under their direction, while they retain overall care management responsibility.
- Provide practical and administrative support to social workers in delivering high-quality care and safeguarding outcomes.
