Practice Nurse in Stockport

We are looking for a motivated and compassionate Practice Nurse to join our friendly and supportive primary care team. The successful candidate will provide high-quality, patient‑centred nursing care, including chronic disease management, immunisations, cervical screening, health promotion, and long‑term condition reviews. You will work collaboratively within a multidisciplinary team, delivering safe, evidence‑based care while contributing to the ongoing development of our practice. Main duties of the job:

• Deliver high‑quality, patient‑centred nursing care within the GP practice.
• Undertake chronic disease management and long‑term condition reviews.
• Provide immunisations, vaccinations, and cervical screening.
• Promote health and wellbeing through patient education and lifestyle advice.
• Perform routine clinical procedures, including blood pressure monitoring, ECGs, and venepuncture where appropriate.
• Maintain accurate clinical records using the practice's electronic patient record system.
• Work collaboratively with GPs and the multidisciplinary team to deliver safe, effective care.
• Participate in clinical audits, quality improvement initiatives, and mandatory training.
• Adhere to professional standards, infection prevention and control, safeguarding, and practice policies.

Key Responsibilities:

• Provide safe, effective, patient‑centred nursing care within own scope of practice.
• Undertake chronic disease management clinics including: Diabetes, Asthma, COPD, Hypertension, Cardiovascular disease.
• Deliver cervical screening in accordance with the NHS Cervical Screening Programme.
• Administer childhood and adult immunisations and vaccinations in accordance with national guidance and Patient Group Directions (PGDs).
• Remove sutures and clips.
• Perform venepuncture, ECGs, blood pressure monitoring, urinalysis and other routine clinical procedures.
• Carry out NHS Health Checks where appropriate.
• Provide contraception advice and family planning services (where trained).
• Promote healthy lifestyles and provide health education and smoking cessation advice.
• Recognise deteriorating patients and escalates concerns appropriately.

Long‑Term Condition Management:

• Undertake structured annual reviews for patients with long‑term conditions.
• Develop and review personalised care plans.
• Support self‑management and patient education.
• Monitor treatment effectiveness and refer appropriately.

Quality and Outcomes Framework (QOF):

• Contribute to the achievement of QOF indicators through effective clinical management and accurate coding.
• Ensure clinical documentation is complete, accurate and recorded promptly.
• Identify patients requiring recall and follow‑up.
• Participate in clinical audits and quality improvement activities.
• Work with the wider team to improve clinical outcomes and performance against QOF targets.

Enhanced Care Coordination (BECCOR):

• Support enhanced care coordination for patients with complex health needs, frailty, and multiple long‑term conditions.
• Work collaboratively with GPs, care coordinators, social prescribers, community nurses, pharmacists, and wider multidisciplinary teams.
• Participate in multidisciplinary team (MDT) meetings where appropriate.
• Assist in developing personalised care plans that reflect patients' clinical and social needs.
• Support proactive care for vulnerable patients to reduce avoidable hospital admissions and improve continuity of care.

Health Promotion:

• Promote disease prevention and healthy lifestyle choices.
• Provide education regarding diet, exercise, smoking cessation, alcohol reduction and weight management.
• Encourage uptake of screening and vaccination programmes.

Clinical Governance:

• Maintain accurate and contemporaneous patient records.
• Participate in clinical audit, service evaluation and quality improvement.
• Comply with NMC Code and professional standards.
• Participate in significant event reviews where appropriate.

Infection Prevention and Control:

• Adhere to infection prevention and control policies.
• Ensure clinical equipment is cleaned and maintained appropriately.
• Report infection risks promptly.

Safeguarding:

• Identify and report safeguarding concerns relating to children and vulnerable adults.
• Complete mandatory safeguarding training.
• Work in accordance with local safeguarding procedures.

Professional Responsibilities:

• Maintain professional registration with the Nursing and Midwifery Council (NMC).
• Maintain clinical competencies and undertake continuing professional development (CPD).
• Participate in annual appraisal and mandatory training.
• Work within own scope of competence and seek advice where necessary.
• Support students and new staff where appropriate.

Health & Safety:

• Comply with Health and Safety legislation and practice policies.
• Participate in emergency procedures, including Basic Life Support.
• Report incidents, accidents and near misses.

General Responsibilities:

• Work collaboratively within the multidisciplinary team.
• Maintain confidentiality in accordance with GDPR and Caldicott principles.

Person Specification:

• Registered Nurse (NMC).
• Experience working within primary care or community nursing.
• Competent in chronic disease management.
• Cervical cytology qualification.
• Immunisation and vaccination training.
• Excellent communication and organisational skills.
• Knowledge of QOF and long‑term condition management.
• Ability to work independently and within a multidisciplinary team.
• Experience using EMIS Web.
• Independent Prescribing qualification.
• Mentor or assessor qualification.
• Experience participating in Enhanced Care Coordination (BECCOR) or Primary Care Network initiatives.
• Knowledge of Enhanced Access services and Quality Improvement methodologies.
• Clinical skills: competent in clinical assessment, health promotion, venepuncture, blood pressure monitoring, and patient education.
• Excellent verbal and written communication skills; ability to build positive relationships with patients and colleagues; ability to communicate sensitively with patients from diverse backgrounds.
• Ability to prioritise workload, manage time effectively, maintain accurate clinical records, and work independently.

• Full UK driving licence (if required for community or PCN‑based roles).
