Your Role
As a Social Care Officer within the East Neighbourhood Social Work Team, you will play a key role in supporting adults and their families to live independently, build resilience and achieve the outcomes that matter most to them. You will undertake person‑centred assessments and support planning under the Care Act 2014, including assessments for basic equipment and Technology Enabled Care (TEC). Working as part of our neighbourhood team, you will contribute to a responsive duty service, help residents access community support and services, and work collaboratively with health, housing and voluntary sector partners to promote positive outcomes and Gloriously Ordinary Lives for the people of Stockport.

We are looking for a caring, motivated and solution‑focused individual who is passionate about supporting adults who face additional challenges. You will have experience of working in a statutory, health, social care or voluntary sector setting, with the ability to build positive relationships and work in a person‑centred way. You will be organised, resilient and able to manage a busy workload while working effectively as part of a team.
