This role is an exciting opportunity to join Stockport Family as a Best Start in Life Coordinator, playing a key role in improving outcomes for children aged 0-5 and their families through the delivery of targeted Early Help, Family Hub and Best Start for Life support.
Working within a multi-disciplinary neighbourhood team, you will provide strengths-based, whole-family support, helping families to overcome challenges, build resilience and improve outcomes in areas including school readiness, communication and language development, social and emotional wellbeing, and positive parenting. You will undertake assessments, coordinate family support plans and act as Lead Professional where appropriate, ensuring children and families receive timely and effective support.
You will work closely with parents, carers, early years providers, health professionals and partner agencies, leading Team Around the Family and Team Around the Early Years meetings to deliver coordinated and outcome-focused interventions. Alongside delivering evidence-based interventions, you will use your professional judgement to assess needs, manage risk, identify emerging concerns and support families to access the right services at the right time. Through relationship-based practice you will help prevent escalation to statutory services and ensure every child has the best possible start in life.
About you
You are a motivated, compassionate and skilled practitioner passionate about improving outcomes for children and families. You have experience working with children aged 0-5 and their families, delivering targeted support, undertaking assessments and coordinating multi-agency interventions. You are confident building positive relationships with families and professionals, including those who may be reluctant to engage with services. You have excellent communication skills, strong professional judgement and the ability to work collaboratively with a wide range of partners to achieve positive outcomes for children.
You bring knowledge of child development, school readiness, safeguarding and early intervention, and experience delivering evidence-based interventions and supporting families with complex and often challenging circumstances. You are organised and proactive, able to manage competing demands, maintain accurate records and use performance information to monitor progress and demonstrate impact. Above all, you share our commitment to inclusion, reducing inequalities and ensuring children and families receive the support they need to thrive.
