The Bellfield Centre is looking for an experienced, dynamic and dedicated Intermediate Care Worker to join our vibrant and supportive team. This is an exciting opportunity for someone with a background in social care who is passionate about making a real and lasting difference to the lives of older people using our integrated Intermediate Care service.

As an Intermediate Care Worker, you will play a key role in supporting older people to regain independence, improve wellbeing and achieve their personal outcomes. You will work as part of a fully integrated health and social care team, providing high‐quality, person‐centred care and support.

  • Has experience working as a Social Care Worker or in a similar care role
  • Is passionate about improving outcomes for older people
  • Understands Intermediate Care and its role in supporting recovery and independence
  • SVQ Level 3 in Social Care
  • Experience delivering professional care and support to older adults
  • Ability to work effectively within a busy and evolving care environment
