We are seeking a highly motivated and values-driven Occupational Therapist to join our newly established Care at Home Review Team. This is an exciting opportunity to shape a dynamic new service that ensures care at home packages remain person-centred, proportionate, and aligned with individual outcomes. As part of the Care at Home Review Team you will be working across both urban and rural communities.

As an Occupational Therapist, you will bring your expertise in functional assessment and environmental adaptation to the review of care at home packages. Working across both urban and rural communities, you will:

  • Undertake outcome focused reviews of care packages, ensuring they remain responsive to changing needs.
  • Apply your professional skills to assess functional ability, independence, and participation in daily life.
  • Identify opportunities for rehabilitation, reablement, and equipment provision that reduce reliance on formal care.
  • Collaborate closely with service users, carers, and multi-agency partners to promote wellbeing, safety, and independence.
  • Contribute to shaping a service that reflects the values of the Health and Social Care Partnership.

We are looking for someone who:

  • Has excellent communication and interpersonal skills, with the ability to build trust and navigate sensitive conversations.
  • Demonstrates sound knowledge of relevant legislation, including Self-Directed Support and Adult Support and Protection.
  • Brings strong person-centred assessment and review skills, with a commitment to strengths-based, outcomes-focused practice.
  • Is confident working autonomously while contributing to a collaborative team culture.
  • Is passionate about continuous improvement and shaping a service that reflects the values of the Health and Social Care Partnership.
