The Bellfield Centre is looking for an experienced, dynamic and dedicated Intermediate Care Worker to join our vibrant and supportive team. This is an exciting opportunity for someone with a background in social care who is passionate about making a real and lasting difference to the lives of older people using our integrated Intermediate Care service.
As an Intermediate Care Assistant, you will play a key role in supporting older people to regain independence, improve wellbeing and achieve their personal outcomes. You will work as part of a fully integrated health and social care team, providing high-quality, person-centred care and support.
We Are Looking For Someone Who:
- Has experience working as a Social Care Assistant or in a similar care role
- Is passionate about improving outcomes for older people
- Demonstrates strong values, compassion and a person-centred approach
- Understands Intermediate Care and its role in supporting recovery and independence
- Has excellent communication and organisational skills
- Enjoys working collaboratively as part of a committed multidisciplinary team
Essential Requirements:
- SVQ Level 2 in Social Care
- Experience delivering professional care and support to older adults
- Ability to work effectively within a busy and evolving care environment
- Candidate's should be registered with the SSSC or in the process of registering

The Benefits:
