Als casemanager ben jij het eerste aanspreekpunt voor cliënten en hun mantelzorgers. Jij geeft advies over het ziektebeeld dementie, denkt mee over passende zorg en organiseert de juiste ondersteuning.
Verantwoordelijkheden
- Je begeleidt cliënten en hun netwerk tijdens het hele ziekteproces, tot aan een eventuele opname.
- Je stemt af met huisartsen, wijkverpleegkundigen, bewindvoerders en andere betrokken zorgprofessionals.
- Je neemt deel aan multidisciplinaire overleggen met andere zorgprofessionals uit de regio.
- Je draagt bij aan de verdere professionalisering van casemanagement dementie binnen Humanitas.
Qualificaties
- Een afgeronde hbo-opleiding Verpleegkunde (hbo-v) of Social Work.
- Het diploma Casemanagement Dementie óf de bereidheid dit via Humanitas te behalen bij de Breederode Hogeschool.
- Empathisch vermogen, respect en begrip voor cliënten en hun familie.
- Zelfstandigheid, maar ook het vermogen om goed samen te werken met collega’s en ketenpartners.
Jouw team
In Hoek van Holland werk je samen met 2 ervaren casemanagers dementie van Humanitas. Je hebt daarnaast overleg en intervisie met collega’s. Zo bouw je mee aan goede dementiezorg.
Wat wij jou bieden
- Een zelfstandige functie met veel verantwoordelijkheid en afwisseling.
- Een jaarcontract voor 24-32 uur per week, met uitzicht op een vast contract.
- Salaris conform CAO VVT: FWG 55 bij afgeronde opleiding Casemanagement Dementie (FWG 50 als je de opleiding nog volgt).
