Harlow North PCN Digital and Transformation Manager
Harlow North Primary CareNetwork are a group of five Practices within Harlow. We jointly deliver services that improve outcomes and provide positive patient experience for our population of 60,000+.
As a Primary Care Network (PCN) our aim is to work collaboratively, enabling our team of Social Prescribers, Care coordinators, Clinical Pharmacists, Nursing Associates and GP Assistants to work together with the Practices. We seek continuous improvement by strengthening our relationship with other key health and wellbeing providers to maximise the impact of our services on quality and outcomes of patient care.
The Practices within the PCN are Addison House, Church Langley, Nuffield House, Sydenham and Old Harlow All practices use Systmone clinical system.
We are a friendly, supportive, democratic, well organised and enthusiastic Primary Care Network. We contain training practices and actively encourage learning and development for all staff.

Stellar Healthcare is a GP Provider company consisting of 22 member practices across Harlow and Epping Forest in west Essex. We work closely with the Practices and PCNs to deliver the best care to patients.
Job responsibilities
Role summary
The Transformation and Development Manager will work with clinical and support teams within a Primary Care Network (PCN) to identify opportunity for and lead service improvements.
The post holder will plan, coordinate, and directly support delivery of evolving projects across the PCN with a view to improving patient access, staff and patient satisfaction, optimising the use of technology and improving the efficiency and sustainability of general practice services.
The post holder will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. The post holder will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits both at individual practice level and at the level of the PCN.
The post holder will work closely with the ICB Primary Care and Digital teams to ensure local transformation plans and activity are aligned to the ICB Primary Care priorities e.g. improving access and PCN development and the ICB Digital Strategy.

The post holder will work to coordinate with and bring in additional specialist expertise when required to deliver transformation plans, e.g., in analytics, evaluation, policy, IT systems, estates etc.
The post holder will help to develop strategy related to transformation and improvement, aligning this with larger scale strategy and initiatives; but will focus their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff, ensuring they address health inequalities and championing diversity and inclusion.

- Experience of successfully delivering change and improvement programmes in a patient facing environment
- A proven track record of communicating and engaging with a wide range of staff and stakeholders – building excellent and trusted relationships
- Significant experience of driving change including translating strategic objectives into tangible plans for delivery
- Ability to use data and insight to both drive change and also to understand and adapt programmes of work
- Ability to work and make decisions autonomously, and to identify and act on challenges to move delivery forward, potentially working to tight timescales
- Ability to understand, interpret and present complex and/or sensitive information
- Ability to interpret qualitative and quantitative data from a range of sources and present it in a clear and concise way to stakeholders.
- Excellent communication skills and an ability to engage successfully with a wide range of people at all levels within the organisation
