Job Responsibilities

• To undertake the assessment of patients and clients with complex care needs, including individuals with undifferentiated and undiagnosed life‑limiting conditions.
• To plan, implement and evaluate specialised programmes of care utilising evidence‑based practice to achieve optimum best clinical practice in palliative care and supportive care.
• To provide specialist advice to staff in managing complex situations, ensuring that patients/clients needs are met and that significant others are supported.
• To independently liaise and communicate effectively with all members of the multidisciplinary team and other agencies involved in the care of the patient/client.
• To initiate risk assessment processes to determine risks to the health and well‑being of patients with complex care needs, taking relevant action to minimise these risks.
• To promote patient/service‑user involvement in the planning, delivery and evaluation of care, respecting their wishes, beliefs and dignity.
• To utilise information and data from a range of sources to identify patient/client issues and guide decision‑making processes.
• To have an active role in education of Health and Social Care (HSC) and outside agencies or charities staff through in‑service training, multi‑disciplinary training programmes, student training and work experience.
• To ensure that professional standards are upheld by staff members working within the postholder’s sphere of responsibility.
• To preserve safety by following and contributing to the development of policies, procedures & guidelines relating specifically to palliative care nursing.
• To be accountable for ensuring that safeguards are in place for patients/service users and to escalate any safety, safeguarding or risk concerns following the appropriate processes.

Qualifications

• Registered Nurse
• Current registration with NMC
• Degree in related subject or demonstrable experience related to clinical area
• Possess a full and valid driving licence; vehicle for business use is required for this post.

• Recognised teaching qualification
• Degree in related subject
• Evidence of post‑registration education relevant to clinical area
• Advanced communication skills training

Experience

• The ability to evidence how HSCs CARE values are applied in practice.
• Evidence of relevant CPD.
• High standard of communication skills, both written and verbal, with the ability to deal with complex and challenging situations in a calm and diplomatic manner.
• The ability to effectively prioritise own workload and that of others.
• The ability to work autonomously and also as part of a multi‑disciplinary team.
• Commitment to self‑development and the development of others.
• Adaptable and responsive to changing service needs.
• Ability to deal with stress in self and others, recognising when support is required.
