Introduction

The Stoughton Senior Center is seeking a compassionate, self-motivated Senior Center Case Manager to support adults 55+ in maintaining independence and enhancing their quality of life.

This full-time position plays a vital part in advocating for older adults, assessing needs, coordinating services, and connecting clients with financial, medical, nutritional, transportation, and housing resources.

Position Summary

This role focuses on supporting older adults by assessing their needs, connecting them with appropriate services, and advocating on their behalf.

The position involves interviewing clients and their support networks to determine required services, planning housing and care options, and ensuring the appropriate use of funding sources.

It also includes evaluating eligibility for programs such as housing, financial assistance, food support, and home-delivered meals, as well as monitoring ongoing services and recommending changes when necessary.

Safety and well-being are central responsibilities, including assessing home environments, reporting abuse or neglect, and making referrals to agencies for issues related to housing, insurance, Medicare/Medicaid, and legal needs.

The role requires strong communication skills to inform clients and the community about available services, maintain relationships, attend relevant meetings, and complete documentation, reports, and outreach materials.

Continuous learning is emphasized to stay updated on resources that support older adults.

Salary Information

This full-time, non-exempt position offers an hourly wage of up to $30.00 and an excellent benefits package, including Wisconsin Retirement, medical, dental, and life insurance.

  • Must be able to utilize a variety of advisory data and information such as financial statements, long-term care assessment information and forms, police reports, assistance and services application forms, referral information, hospital discharge information, elder law manual, Medical Assistance manuals, case management manuals, procedures, guidelines and non-routine correspondence.
  • Ability to communicate orally and in writing with clients, clients’ families/guardians, Senior Center personnel, medical care providers, nursing home personnel and social workers.
  • Must be competent in Microsoft Office programs, other software applications, and the Internet.
  • Conditions of Employment
  • A valid Wisconsin driver's license is required at hire. A well-functioning automobile to be used extensively.
  • Must pass a background check, driver’s license check, pre-employment physical, and drug screen.
  • Environmental Adaptability
  • May be exposed to cigarette smoke on home visits.
  • May be exposed to pets (sometimes can be unfriendly)
  • May encounter unpleasant living conditions upon home visits.
  • May be exposed to infected body fluids. May be exposed to potentially harmful cleaning agents.
  • Ability to lift 25 pounds.
  • Required to change from sitting, standing, and walking positions repeatedly.
  • Repetitive motion as required for computer keyboard use and handwriting.
  • Need to be in and out of the car in temperature extremes.

Qualifications

Education, Training, Certifications & Experience

  • A four-year degree in a human service-related field or comparable experience.
  • Experience working with older adults.
  • Knowledge of older adult programs and related resources.
