The City of Stoughton’s Senior Center is seeking a compassionate, self-motivated Senior Center Case Manager to support adults 55+ in maintaining independence and enhancing their quality of life. This role advocates for older adults, assesses needs, coordinates services, and connects clients with housing, medical, nutrition, transportation, and other resources.
