Join Our Team as a Support Facilitator (Home Care) - Make a Real Difference Every Day! Staffordshire County Council’s Provider Services team is looking for dedicated Support Facilitators to deliver person‑centred reablement care in the community. You will be part of an innovative and empowering service that supports individuals to thrive in their own homes. Applicants must live within 30 minutes of our base location DE14 1AH to ensure timely response and continuity of care. This role requires travelling within the community, primarily East Staffordshire, and so a full UK driving licence and access to your own vehicle is essential.

We have a part‑time position available with a range of shifts between 7:00 AM and 10:00 PM, including weekends. All early shifts will start at 7:00 AM. You may be required to work other days to complete training.

  • Undertake manual handling tasks as required.
  • Drive (using your own vehicle) to deliver care across the local area.
  • Complete interventions using Assistive Technology.

What We Need From You

  • Caring and compassionate nature, with a genuine desire to make a positive difference in people’s lives every day.
  • Strong communication skills and the ability to build trust and positive relationships with individuals, families, and colleagues.
  • Flexibility and resilience, adapting to changing needs and working patterns while maintaining a positive outlook.
  • Practical skills and reliability, including a full driving licence, physical ability for care tasks, and a commitment to learning and development.
  • Experience in social care (NVQ Level 2 or equivalent).
  • Access to a vehicle for work purposes.
  • Caring, patient, and committed to empowering people to live independently.
  • Ability to work flexibly across days, evenings, weekends, and bank holidays.
  • Physical ability to support personal care and manual handling tasks.
  • Great communicator with a positive outlook and ability to stay calm under pressure.
  • Lives within 30 minutes of our base location DE14 1AH.

Benefits

  • Full paid training and ongoing development to enable you to pursue your career aspirations.
  • BLOCK pay – you will be paid for the whole shift duration, including travel time.
  • Travel reimbursement at £0.45 per mile, paid from Burton Library.
  • Supervision and support to enable you to provide excellent care.
  • Position is predominantly based in the Burton / East Staffordshire area with an expectation to travel across North Staffordshire to support other areas as business needs dictate.

This post is subject to an enhanced DBS check and will require a pre‑employment health assessment as part of the onboarding process.
