Overview
You will work within the Neurodivergent team to support Neurodivergent Adults with personalised Care Act assessments and support plans.
You will work within a new bespoke team set up to develop skills and independence and to lead fulfilling lives, and with the Consultant Social Worker for Autism for bespoke advice, guidance, and support.
Responsibilities include completing strengths-based assessments and reviews, meeting statutory functions under the Care Act (2014) including assessment, case management, and S42 safeguarding enquiries.
You will create detailed support plans using local resources for Neurodivergent citizens of Staffordshire within their local communities.
You will utilise the Mental Capacity Act (2005) including capacity assessments, applications to the Court of Protection, and other court-directed work.
You will record all case management actions and key decisions on the case recording system.
You will analyse information to understand risks and develop strategies to explain, avoid, or manage risk appropriately.
You will participate in quality assurance processes, including peer and reflective supervision, and use research findings and case discussions to inform and develop your practice.
You will develop and co-produce support plans with the client to maximise independence for adults with care and support needs.
You will participate in development and training opportunities to progress professional development such as Practice Educator or Best Interest Assessor roles.
Qualifications and Experience
- Be a qualified Social Worker with experience in Adult Social Care and have professional registration with Social Work England.
- Have a proven track record of a person-centred and asset-focused outlook and high professional standards.
- Have sound knowledge and experience of current legislation and its application to practice in a social care setting.
- Have applied knowledge of the Care Act and the Mental Capacity Act and experience of undertaking safeguarding enquiries.
- Have experience of working in complex situations using professional judgement and a range of interventions.
- Ability to communicate clearly in a jargon-free, courteous, calm, and professional manner.
