Responsibilities
- Have a passion for supporting adults to achieve their full potential and want to gain further experience in improving outcomes.
- Support adults from the very start of their journey, reviewing their goals and ambitions.
- Promote independence and opportunities, ensuring that these have positive outcomes.
- Undertake robust assessments, support planning, review and case management to support adults with a learning disability and/or autism ensuring that they, their families/carers and the people important to them are supported to live the life that they want to live.
You will need a professional social work qualification (DipSW, CQSW, CSS) or other social work qualification certified by Social Work England.
