We are seeking a Clinical Pharmacist to join a GP practice in Abingdon, Oxfordshire on a temporary-to-permanent basis. This is a fantastic opportunity to work within a supportive primary care team, leading on medicines optimisation, patient care, and clinical prescribing.
Key Responsibilities:
- Prescriber (or training to become one) working alongside GPs
- Manage patients with chronic conditions and complex medication regimes
- Conduct structured medication reviews, including STOMP initiatives
- Lead on medicines optimisation, repeat prescribing, and therapeutic drug monitoring
- Support long-term condition clinics, home visits, and patient consultations
- Advise on cost-effective prescribing and quality improvement
- Liaise with multidisciplinary teams, community/specialist pharmacists, and secondary care
- Ensure compliance with NICE and CQC standards
Requirements:
- Registered Pharmacist with experience in primary care
